5. If you didn't elect to prompt your driver for consent forms on the previous page, you'll need to have all required release docs ready for upload.

Driver Watch is a carrier based reporting system on commercial truck drivers. Carriers nationwide have the ability to submit Driver Watch reports on drivers based on direct experiences with the driver. The Driver Watch report was created to standardize the method of which carriers can report bad drivers and warn other carriers about these drivers.
Your reports are protected by two-step verification. A one-time code will be sent by text or phone call to access your documents.
Certification of Permissible Purpose
By submitting this request, I certify that I am authorized to request this report on behalf of my organization and that my organization has a permissible purpose to obtain this report under the Fair Credit Reporting Act (FCRA) and the Driver’s Privacy Protection Act (DPPA), including employment-related purposes under 18 U.S.C. § 2721(b)(9).
I authorize Driver-411 to act as my organization’s authorized agent for the purpose of obtaining this motor vehicle record report from its approved reporting providers. I certify that this request is being made solely for the pre-employment screening or ongoing employment monitoring of a commercial motor vehicle driver and that my organization will use the report only for lawful employment purposes.
I understand that Driver-411 will obtain the required driver authorization prior to ordering and/or furnishing the report. I agree that any information received will be maintained securely and used only for the stated permissible purpose in compliance with all applicable federal and state laws, including the FCRA, DPPA, and applicable FMCSA regulations.
